Crossroads is a family of professionals dedicated to providing the most accessible, highest quality, evidence-based medication assisted treatment (MAT) options to combat the growing opioid epidemic and helping people with opioid use disorder start their path to recovery. This comprehensive approach to treatment, the gold standard in care for opioid use disorder, has been shown to prevent more deaths from overdose and lead to long-term recovery. Our clinics are all outpatient and office-based, with clinics in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia. As an equal opportunity employer, we celebrate diversity and are committed to an inclusive environment for all employees and patients.# # ***Day in the Life of a Patient Navigator**** Greeting patients and answering clinic phones.* Assist patients in understanding their treatment options, and the resources.* Serve as an essential link between patients and all other care.* Facilitate appointments for consults and support services within established service standards, including social workers, physician, etc.* Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.* Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management.* Work with staff and others to collect data, track outcomes, and support strategic planning processes.* Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, social services.* Assist Center in stocking supplies and Patient Ambassador team.* Other duties as assigned. # ***Education and Experience requirements***## *Must hold one of the following credentials/experiences:** RN, LPN, or BSW* Bachelor’s Degree ***WITH*** at least 2 years of lived addiction experience.* Associate's degree or CMA ***WITH*** 4 years of lived addiction experience. # ***Schedule**** Monday (Belvidere) 8:30am-1:30pm* Wednesday (Stanhope) 9:30am-2:00pm* Thursday (Belvidere) 11:30am-5:30pm* Sunday (Stanhope) 9:00am-1:00pm# ***Position Benefits**** **Have a daily impact on many lives**.* **Excellent training if you are new to this field.*** **Mileage reimbursement (if applicable)** Crossroads matches the current IRS mileage reimbursement rate.* **Community events that promotes belonging and education**.
