Accompany Health is seeking a Health Advocate, Float to provide in-home and virtual care coverage across patient panels. You will step in when regular team members are unavailable, delivering high-quality care and ensuring continuity of service. Responsibilities include onboarding visits, routine follow-ups, and urgent social care needs.
