Key Responsibilities\n

  Conduct thorough assessments of patients to identify their mental health status and any progress toward therapeutic goals, utilizing tools consistent with evidence-based practices, such as the Diagnostic and Statistical Manual of Mental Disorders.

  Develop and implement individualized treatment plans that encompass culturally responsive strategies and address the unique needs of each patient.

  Continuously monitor and evaluate patient progress and adjust treatment strategies as necessary to ensure optimal outcomes.

  Prepare and maintain accurate and timely treatment records in compliance with regulatory requirements, using current electronic health record (EHR) technology.

  Work collaboratively with patients, their families, and a multidisciplinary team to create comprehensive care plans that prioritize safety and symptom management.

  Act as a liaison between patients and external treatment providers, coordinating referrals and establishing successful working relationships with community resources.
