Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in the specified territory. This position involves providing outpatient therapy to geriatric patients, with a focus on home-based care. The role requires timely documentation, flexibility with territory, and adherence to clinical standards.

Job Details:

• Shift Hours: 8 hours per day

• Minimum Hours Per Week: 40 hours

• Start Date: 10/05/2026

• End Date: 01/04/2027

• Required Qualifications: Valid license in hand, minimum of 1 year of relevant experience, graduate degree in Physical Therapy, proficiency with Homecare Homebase (HCHB), OASIS experience required for staff completing SOC (Start of Care) in Home Health, BLS certification, recent outpatient or home health experience, recent.

• Preferred Qualifications: Experience with geriatric outpatient therapy, familiarity with body mechanics, post-operative orthopedic recovery, cognitive disorders, and neurological conditions.

• Additional Information:

  • Patient visits average 5-7 patients per day.
  • No first-time travelers accepted.
  • On-call rotation required, 6th weekend on-call schedule.
  • No cell phone provided; iPads supplied.
  • Mileage reimbursed at IRS standard rate.
  • All travelers must have active drivers licenses and personal transportation.
  • Dayshift schedule.
