POSITION SUMMARY:

The Black Infant Health (BIH) Program is meant to address the problem of poor birth outcomes and health disparities that affect African American women and their babies. The BIH program features a standard set of services that features group intervention and social service case management to help reduce stress, build resilience, promote healthy behaviors, and increase social support. The BIH Program Manager will have day to day responsibility for leadership of the BIH program and oversee all program team members and partners. This includes developing relationships with community-based collectives, community-based organization partners, and management of the Community Advisory Board. The position also includes management of project timelines and deliverables for timely and effective execution of the Work Plan and responsibility for project reporting. Ongoing and timely communication with Alameda County Public Health Department and other community partners will be expected as well. This position supports Alameda County’s BIH Program Area 2, covering families who reside in West Oakland, Emeryville, North Oakland and South Berkeley.

1.Education: Bachelor’s Degree from an accredited college or university program in Social Work, Public Health, Nursing, Education, Health Education, or other health related or social science field plus five years of management experience in a health/public health or social service setting is required; or Master’s Degree from an accredited college or university program in Social Work, Public Health, Nursing, Education, Health Education, or other health related or social science field.

2.Experience: Lived understanding of African American culture and history, and general working knowledge of family systems, community trauma, and community organizing or advocacy. Experience in serving Black birthing and parenting individuals, providing family support, and offering home visiting services is key. At least five years of professional administrative experience in public health or a related field, including two years of supervisory experience.

3.CA Department of Public Health Approval: The BIH Program Manager must be approved by the California Department of Public Health (CDPH)/Maternal Child and Adolescent Health (MCAH) BIH Program Consultant prior to appointment.

4.Black Infant Health Ambassador: Fully embraces and supports the Black Infant Health Governing Concepts of culturally relevant, client-centered, strength-based, and cognitive skill-building approaches.

5.Cultural Competence: Demonstrates cultural competence. Demonstrates ability to implement and maintain a culturally relevant BIH Program that recruits, trains, and retains staff who reflect and respond to the values of the African American community served by BIH.

6.Knowledge of Public Health: Demonstrates knowledge of the following: women’s health, including prenatal and postpartum health; life course perspective; infant behavior and development; health education, including breastfeeding, nutrition, and physical activity; and local community and social services.

7.Advocacy and Outreach Skills: Essential qualifications focus on strong community engagement, population-based assessment, and evidence-based service delivery skills, particularly in women's health, infant behavior, health education, and addressing health inequities affecting the Black community. Advocacy and outreach skills will be essential to promote health equity by implementing health programs and health policies that will benefit the community’s health through motivational training.

8.Interpersonal and Leadership Skills: Possess strong leadership skills. Demonstrates the organizational and interpersonal skills needed to communicate at different levels and work in complex situations. Reflects basic skills of the Health Resources and Services Administration Maternal Child Health Bureau Leadership Competencies 1-8 (leadership.mchtraining.net).

9.Computer Skills: Basic computer skills, and capable of using software and technology tools used to manage health data and reporting tools (Excel, Microsoft word, etc.). Experience in managing and tracking health data, navigating Epic and Tableau a plus.

10.Other: Should be able to remain calm and courteous in a high stress environment. Should be able to work independently with minimal direction.

1.Program Management: Responsible for the management and coordination of local BIH program activities and staff. Maintains confidentiality and adheres to Health Insurance Portability and Accountability Act (HIPAA) regulations. Provides supervision and professional development for staff; including assignment responsibilities and preparing performance evaluations. Prepares the BIH program budget and monitors expenditures. Prepares administrative, financial documents and transactions for completion, and gathers and compiles information for biannual reports.

2.Conduct community outreach and need assessments. The BIH program manager will plan, develop, and provide oversight outreach campaigns to enroll pregnant African American women into the Black Infant Health program. Will conduct needs assessments and guide the research, and analysis of statistical and other data to report healthy birth outcomes and effectiveness of the BIH program. Will develop outreach and program literature to raise awareness of African American infant and maternal mortality rates.

3.Program reporting and grant writing. Manages BIH scope of work to meet annual objectives; evaluates program needs and develops effective program activities and services to educate and empower clients in meeting their healthcare and related needs. Research and write grant proposals and agenda reports.

3.Clinical care referrals and patient care tracking[RJ2.1]. Will be able to follow standard protocols to make referrals to both internal and external care provider entities. Will create and maintain a registry of BIH patients, using a robust tracking system (Epic Patient List) that can follow program participants based on panels belonging to individual primary care providers or by condition.

4.Community engagement/advocacy. Will actively engage with community members while implementing health programs by encouraging active participation from individuals and requesting feedback for continuous improvement. Coordinate with the medical staff and care teams to ensure that case management services are provided to patients with complex medical and/or psychosocial problems while ensuring health professionals provide culturally sensitive care (inpatient/outpatient referrals, appointments, scrubbing of charts, care plans, etc.). May attend meetings with county-wide agencies to coordinate health services for low-income families.

5.Other Duties as Assigned. Will attend and actively participate in trainings and care coordination meetings. This includes, but is not limited to, assisting other programs and providing back-up for other team members.

1.Attends work regularly and punctually; is readily accessible during work hours to peers; limits absences from own duty stations and limits visits and activities which distract others from their performance of duties.

2.Willingness and ability to travel to different Baywell clinics to perform work and meet with providers, patients, and staff.

3.Works cooperatively with immediate supervisor, with department members, and with other Baywell Health staff persons, as required.

4.Treats all patients / clients / visitors with respect and courtesy.

5.Remains competent in skills required for position.
