This Home Health Physical Therapist position is based in the Reading, Pennsylvania area, offering the opportunity to live and work in a community that combines a welcoming, close-knit feel with convenient access to larger cities. The role focuses on providing skilled Physical Therapy services to patients in their homes, allowing you to develop meaningful, one-on-one relationships and directly see the impact of your interventions on a patient’s ability to function safely in their usual environment. You will evaluate and treat adult and geriatric patients with a variety of conditions, including post-surgical orthopedic cases, neurologic diagnoses such as stroke, and complex medical conditions requiring strength, balance, and endurance training. A typical day includes traveling to patient residences within the Reading and greater Berks County area, performing initial and follow-up assessments, developing individualized treatment plans, and implementing interventions such as therapeutic exercise, gait and transfer training, home safety evaluations, and education for patients and caregivers. You will document visits in an electronic medical record, coordinate closely with nursing and other therapy disciplines, and participate in interdisciplinary case conferences to ensure cohesive patient care. Patient visits are generally scheduled throughout the day with time allotted for documentation, route planning, and communication with the care team. Productivity expectations are designed to balance visit volume with high-quality, patient-centered care. Because care is delivered in the home, you have the flexibility to organize your schedule while maintaining required visit and documentation standards. You will work with a supportive team culture that values collaboration, professional growth, and evidence-based practice.
