Key Responsibilities\n

• Conduct thorough assessments and evaluations to determine the clinical needs and treatment goals of patients receiving care.

• Facilitate effective communication between patients, their families, and other professionals involved in the care process to ensure a coordinated approach to treatment.

• Develop detailed treatment plans, including schedules for appointments, home visits, and observation sessions, tailored to the needs of individual clients.

• Conduct in-home observations when necessary, coordinating visits and gathering relevant data to adjust therapy plans as needed.

• Ensure accuracy and adherence to the program strategy while actively participating in interdisciplinary team meetings to review progress and treatment outcomes.

• Prepare and submit monthly and quarterly progress reports that evaluate individual patient progress and inform future planning.

• Arrange and deliver training sessions for staff and team members to enhance program execution and compliance with documentation standards.

• Manage patient records effectively, ensuring that all information is readily accessible and maintained in compliance with legal and ethical standards.

• Act as a liaison between the mental health community and the general public to help individuals navigate available resources and achieve desired developmental goals.

You will have the opportunity to work with a diverse client base, catering to a variety of age groups including both youth and adults. Qualifications\n

• An advanced degree (Master's or Doctorate) in Counseling, Psychology, or a closely related field.

• An active and unrestricted Professional Counselor (LPC) license in the state of Colorado.

• At least two years of professional counseling experience, with a preference for clinical settings.

• Demonstrated specialization in family therapy, couples therapy, substance misuse rehabilitation, or trauma-informed care is highly regarded.
