Your Role and Responsibilities

• Performing thorough patient evaluations, ordering necessary tests, and creating tailored care plans
• Identifying and managing acute, chronic, or episodic health conditions, working collaboratively with physicians or independently as permitted
• Instructing patients and their families on healthy lifestyle choices, preventative care, and managing ongoing health issues
• Prescribing medications and therapies according to your practice authorization
• Engaging in patient follow-ups and overseeing chronic care management
• Working cross-functionally with other healthcare professionals to provide holistic and streamlined care
• Accurately documenting all patient interactions within electronic medical record (EMR) systems

Qualifications and Certifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree
• Successful completion of an accredited Nurse Practitioner training program

Licenses and Credentials Required

• Valid Nurse Practitioner licensure in the state of Arizona
• National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or similar specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to achieve within 90 days)
