As part of a collaborative primary care team, the LPC will engage in regular communication with both clinical and administrative staff to effectively address patient needs. Key Responsibilities\n

• Conduct thorough patient intakes including diagnostic and functional evaluations to establish treatment goals.

• Provide brief, targeted interventions for patients requiring immediate mental health support.

• Collaborate with primary care providers to identify and manage mental health and psychosocial issues effectively.

• Regularly follow up with patients to assess their progress and adjust treatment plans as necessary.

• Develop and implement relapse prevention strategies tailored to individual patient needs.
\n
• Monitor and coordinate health services related to behavioral health care in collaboration with other treatment providers.

• Document patient progress in treatment charts, ensuring all data is current and accurate per clinical standards.

• Educate patients on self-management techniques and assist them in their compliance with medical treatments prescribed by their primary care provider.

• Advise clinic management and team members on behavioral health services and potential areas for program improvement.

• Uphold the ethical standards set forth in the Code of Ethical Conduct for Clinical Professionals.

The LPC will serve a diverse patient demographic characterized by varied socio-economic backgrounds and age groups. Qualifications\n

• A minimum of a Master’s degree in Clinical Counseling or a related field.

• Possession of a current Licensed Professional Counselor (LPC) license.

Preferred Experience\n

• Prior experience in a Federally Qualified Health Center (FQHC) setting is a plus.

Compensation and Benefits\n

Yakima Family Health Services offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
