Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to the unique needs of each patient, focusing on their mental health, substance abuse, and medical requirements, to devise effective treatment plans.

• Engage in individual and group counseling sessions aimed at improving emotional health and coping strategies for patients and their families, while maintaining accurate documentation of all interactions.

• Coordinate with local resources and community agencies, facilitating referrals for social work support and necessary services, including medication assistance programs to ensure patients receive critical support.
\n
• Develop and implement data collection processes in collaboration with Quality Improvement teams, enhancing service delivery and measuring patient outcomes.
\n
• Facilitate crisis intervention sessions and make independent judgments regarding the severity of patient issues, recommending appropriate referrals and care strategies.

Patient Population / Client Focus\n

The primary clientele includes individuals in hospice care and their families, facing emotional distress related to terminal diagnoses and end-of-life decisions. You will address various issues such as grief, anxiety, and family dynamics, facilitating coping mechanisms and support strategies to enhance the quality of life for both patients and their loved ones.

Qualifications\n

• A Master's degree in Social Work (MSW) with a current and valid Licensed Clinical Social Worker (LCSW) designation is mandatory.

• Minimum of three (3) years of post-licensure experience, with a strong emphasis on mental health services.

• Expertise in counseling techniques, couple and family therapy, crisis intervention, and knowledge of relevant evidence-based practices.

• Demonstrated proficiency in making autonomous clinical decisions regarding patient care and treatment options.

Preferred Experience\n

• At least two (2) years of experience in a mental health or clinical setting, particularly in hospice or palliative care.

• Proven skills in facilitating group therapy sessions and developing therapeutic plans tailored to diverse patient needs.

Compensation and Benefits\n
