As part of our Community Health Clinic, you will engage with interdisciplinary teams to collaborate on patient care. Key Responsibilities\n

• Conduct ongoing assessments of patients to adequately address their mental health needs, ensuring appropriate care and treatment plans are established.

• Facilitate clear communication between supervisors, direct care team members, family members, and guardians to discuss the progress and challenges faced by patients.
\n
• Provide supervisors with a tentative schedule of appointments, home visits, and observations that align with the therapeutic goals of individual clients.
\n
• Coordinate home visits to gather essential data through direct observations, chart reviews, and staff interviews, fostering a comprehensive understanding of each client's situation.
\n
• Ensure that the implementation of treatment plans is both effective and appropriately monitored to achieve optimal outcomes for clients.
\n
• Actively participate in Interdisciplinary Team Meetings and relevant discussions to foster collaboration and enhance patient care.
\n
• Complete detailed monthly and quarterly progress notes to document the evolving needs of clients and to support continuity of care.
\n
• Organize and lead training sessions for staff to ensure adherence to program protocols and enhance the competency in utilizing documentation systems and therapeutic techniques.
\n
• Provide ongoing follow-up through interviews, home visits, and observations to ensure that clients continue to receive necessary support tailored to their evolving needs.
\n
• Maintain meticulous records to guarantee the availability of information to relevant stakeholders and to improve service delivery.
\n
• Serve as a liaison with the public and mental health community to support developmental goals and enhance awareness of services available.
\n
• Contribute to developing procedures for training materials in order to refine and improve program effectiveness for clients.

Patient Population / Client Focus\n

In this role, you will work with a variety of clients, including adults and youth, who are faced with a range of mental health challenges such as anxiety, depression, trauma, and substance misuse. Your ability to adapt treatment strategies to meet the unique needs of each individual is critical in achieving positive client outcomes.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.

• An active and unrestricted license as a Professional Counselor (LPC) in Arizona is essential.
\n
• A minimum of two years of experience in a counseling role, ideally with a focus on clinical practice.

• Preferred specialization in areas such as family therapy, couples therapy, trauma, or substance use disorder treatment.
\n
• Thorough understanding of legal and ethical standards governing the practice of counseling.
\n
• Demonstrated ability to communicate effectively and establish rapport with diverse clientele.

• Experience working collaboratively within a multi-disciplinary healthcare team is highly valued.

• Proficiency in electronic medical record (EMR) software and related technologies.

Preferred Experience\n

Candidates with specialization or significant experience in family therapy, couples therapy, trauma-informed care, and substance misuse treatment are highly preferred. Additionally, familiarity with community health frameworks and outreach initiatives will contribute significantly to the success of this role.
