Key Responsibilities\n

• Assist patients in accessing a variety of community resources such as support groups, vocational rehabilitation services, and housing assistance programs to ensure a smooth transition back into community life.

• Educate patients and their families about available community resources and post-rehabilitation services to empower them in their recovery journey.

• Participate in discharge planning meetings, facilitating the transition of patients to home care or other appropriate settings to ensure continuity of care and support.

Qualifications\n
