What You'll Do

• Provide outpatient primary care for geriatric patients with complex health needs
• Manage chronic conditions and perform evaluations tailored to senior health
• Educate patients on preventative care and chronic disease management
• Document patient visits accurately within the Epic EMR system
• Collaborate with a dedicated healthcare team to improve patient outcomes
• Participate in on-call coverage, including after-hours and weekends (approximately one week per year)
• Support the clinic's goal of delivering person-centered geriatric care

What You Need

• Board certification in Internal Medicine or Family Medicine
• At least one year of post-training outpatient experience in geriatrics or primary care
• Strong clinical assessment and documentation skills
• Ability to handle a broad spectrum of geriatric health issues

• Experience managing chronic and complex geriatric conditions
• Comfort with outpatient primary care procedures and evaluations
