Key Responsibilities\n

• Assist primary care providers in identifying, treating, and managing patients' mental health and psychosocial issues, thereby contributing to the overall health of individuals.

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, providing essential insights and recommendations for individualized treatment plans.

• Deliver brief, targeted interventions for patients requiring mental health support, ensuring that they receive appropriate care tailored to their unique circumstances.

• Make informed recommendations regarding the triage of patients to either remain in primary care or be referred to specialized mental health facilities.

• Engage in follow-up efforts with patients, monitoring their progress and adjusting treatment approaches as necessary for optimal outcomes.

• Develop relapse prevention strategies to help patients maintain consistent well-being and cope with life’s challenges effectively.

• Collaborate to identify at-risk patients and design preventive action plans addressing their potential worsening conditions.

• Document patient progress comprehensively, maintaining accurate treatment records to keep all team members informed.

• Provide patients with the necessary self-management techniques and educational resources to empower them in their treatment and recovery journey.

• Conduct assessments and consultations with clinic management and team members, driving improvements in behavioral services.

Qualifications\n

• Master’s Degree in Clinical Counseling from an accredited institution.

• A valid Licensed Professional Counselor (LPC) license in Texas.

Compensation and Benefits\n

Mountain Springs offers a competitive salary range of USD 87,000 – 105,000 per year, commensurate with experience and qualifications. Additionally, we provide a SIGN-ON BONUS for successful candidates who join our team.
