Job Details:

Job Title: CWHH-Physical Therapist

Location: Lady Lake, FL

Setting: Home Health

Pay Range: 2551.20 - 2678.76

Contract Length: 13 weeks.

Candidate Type: Traveler

Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting.

Responsibilities:

• Perform clinical assessment to identify patient needs and determine appropriateness of care.

• Document, review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical records; revise the POC based on evaluation and ongoing assessment data.

• Collaborate with interdisciplinary team and work in consultation with physicians.

• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and assessments.

• Assist and instruct patients and families in transfer techniques and use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances.

• Educate patients and families regarding therapeutic procedures, considering the patient’s age, condition, culture, and learning readiness.

• Apply infection control concepts and universal precautions during patient care activities.

• Accept clinical assignments appropriate to education and competence to ensure quality patient care.

• Perform other related duties as assigned.
