Overview

Responsibilities
- Deliver direct nursing care to patients in a home hospice setting using the nursing process; monitor and respond to changes in patient condition.
- Assess, plan, implement, and evaluate care; coordinate with interdisciplinary team to promote comfort, safety, and quality of life.
- Administer medications, manage therapies, monitor vital signs, and respond to emergencies; supervise and delegate tasks to assigned nursing staff as appropriate.
- Provide education and supportive care to patients and family members; participate in discharge planning and transitions of care as needed.
- Maintain accurate documentation in the electronic health record; ensure adherence to safety, privacy, and regulatory standards.
- Collaborate with treating providers, social workers, and other disciplines to optimize care delivery and patient outcomes.

Requirements
- Active RN licensure in the state of practice (California) required
- BSN degree preferred; not required
- CPR or BLS certification from an approved vendor required; BLS certification also required by facility policy
- Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification required
- Hospital Fire and Life Safety (HLFS) certification required
- Minimum of two years of recent RN experience in the field; Epic experience required
- Ability to work in a home healthcare – hospice setting and participate in a 7:00 PM–7:30 AM night shift
