Key Responsibilities\n

• Conduct thorough, ongoing assessments of clients to create clear and actionable treatment plans tailored to their specific needs.

• Facilitate effective communication among supervisors, direct care staff, family members, and other stakeholders to monitor client progress and adjust treatment as needed.

• Provide a structured schedule for appointments, home visits, and observations to enhance clients' therapeutic progress and treatment adherence.

• Coordinate home visits, including completing chart reviews and interviewing clients and their families to ensure holistic support.

• Oversee the implementation of personalized care plans, ensuring that all therapeutic interventions meet established goals.

• Engage actively in Interdisciplinary Team Meetings, collaborating with fellow professionals to enhance collective patient care strategies.

• Document progress through monthly and quarterly notes, ensuring compliance with legal and ethical standards while enabling seamless communication across the healthcare team.

• Organize and lead training sessions to develop staff competencies in executing care plans and maintaining accurate documentation.

• Deliver personalized follow-up care through interviews and home visits to assess ongoing client needs and support recovery.

• Act as a liaison with the public and local mental health community, promoting resources and supportive programs for individuals and families.

• Contribute to the creation of new training materials and procedures aimed at improving therapeutic outcomes and program effectiveness.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, Social Work, or a related mental health field.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Texas.
