Develop and implement plans to help patients reach maximum performance within their capabilities. Perform initial admissions/evaluations and, where appropriate, supervise Physical Therapy Assistants. Document observations, clinical findings, problems, skilled interventions, goals, and discharge plans; monitor progress toward goals. Notify the assigned Clinical Team Manager of OASIS scores within established timelines. Coordinate care with patients, families, and other members of the health care team to align with care plans and patient goals. Requirements One year of Home Health experience Ability to work effectively in a home health setting and manage patient-centered care Capability to perform initial admissions/evaluations and contribute to discharge planning Proficiency in documenting clinical findings, interventions, goals, and discharge plans with timely OASIS submission Flexible scheduling to accommodate patient needs, including a weekend requirement as described Benefits and Support Guaranteed 40 hours per week Weekly pay range stated above 26-week contract with travel assignment status Dedicated recruiter and ongoing travel support throughout the assignment #J-18808-Ljbffr Healthforce
