Key Responsibilities\n

• Deliver client-centered counseling sessions addressing academic, professional, and personal concerns, ensuring comprehensive support.

• Provide medication adherence assistance by scheduling reminders, monitoring compliance, and organizing timely refills in collaboration with medical professionals.
\n
• Facilitate transportation for clients, including the use of personal and agency vehicles, and offer guidance on public transportation options to ensure their attendance at medical appointments and sessions.
\n
• Support clients in everyday living skills, including meal planning and preparation, ensuring they develop independence and confidence in their abilities.
\n
• Maintain high standards of care by consistently reviewing client goals, monitoring progress, and adapting strategies to maximize overall satisfaction and outcomes.

• Adhering to all relevant legal, regulatory, and contractual obligations, ensuring full compliance with program certifications and licensing requirements.
\n
• Collaborate with a multidisciplinary team by providing timely updates and fostering open communication across the board.
\n
• Ensure accurate record-keeping by maintaining detailed client documentation, including session notes, progress reports, and logistical records such as mileage.

• Act as a mentor and positive role model, actively demonstrating and reinforcing appropriate safety protocols.

Your role will focus on serving a diverse group of clients, including adolescents, young adults, and families, each facing unique challenges. Qualifications\n

• A Master’s degree in fields such as counseling, clinical psychology, social work, marriage and family therapy, or related areas is required.

• Licensure as an LCSW, LMFT, LPCC, or equivalent is mandatory; unfortunately, applicants with an associate license cannot be accepted due to Medicare billing requirements.

• Proficient computer skills, particularly within Microsoft Office Suite, are necessary for documentation and communication.

• The ability to operate independently and manage a caseload effectively in a fast-paced environment is essential.

Preferred Experience\n

• Prior experience working with geriatric populations is preferred but not essential.
