Case Coordinator - Bilingual English/SpanishGrowing Homecare company providing services throughout the NYC area is looking for a Bi-Lingual Case Coordinator. This position will:- Coordinate and Schedule appointments for Community Health Workers (CHW)- Ensure all appointments are accurately updated in Master list and corresponding notes are maintained- Collect files, assign members, and monitor performance of Community Health Workers (CHW) to ensure they are able to complete their scheduled visits- Manage client accounts and maintain accurate recordsRequirements:One Year Homecare experience preferredBi-Lingual - English/SpanishWe provide equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
