Guides clients, consumers and their families through the care continuum establishing contact upon referral and continuing contact throughout care and to self- management and/or active treatment. Required Minimum Knowledge, Skills, and Abilities (KSAs) Education: Bachelor’s Degree in Social Work or related field preferred. License/Certifications: N/A Experience: Two years of experience working with behavioral health consumers and families preferred. Experience in a team based setting and collaborating with multiple providers preferred.
