Helping Hand is seeking a dedicated and compassionate Case Manager/QIDP to join our Case Management Team. In this role, you'll provide person-centered support that helps individuals pursue their goals, exercise informed choice, and live safely and independently in the least restrictive environment. You'll coordinate services, advocate for individual needs, and collaborate with families, service providers, and community partners to promote continuity and quality of care.
SCHEDULE AND COMPENSATION:
- On-site position, Monday-Friday
- 40 hours per week.
- Some flexibility is needed during varying hours, including evenings and weekends in accordance with the position’s needs.
- Salary range $55,000-$58,000year (exempt).
- Salary based on skills and experience.
- Up to 8% annual bonus potential.

WHAT YOU'LL DO:
Person-Centered Planning and Advocacy
- Develop, coordinate, and monitor individualized service plans based on each person's needs, preferences, hopes, and goals.
- Promote individual rights, informed choice, community participation, personal safety, and independence.
- Advocate for necessary services and supports in the least restrictive and most integrated setting.
- Develop individualized or nontraditional programming when needed.
Service and Care Coordination
- Serve as a liaison among individuals, families, internal departments, IDT members, ISC agencies, medical providers, schools, hospitals, therapists, DHS, and other community partners.
- Coordinate and lead IDT meetings, medical appointments, home visits, and other service-related meetings.
- Assist individuals with accessing and maintaining benefits and resources, including SSI, SSDI, Medicaid, Medicare, SNAP, transportation, employment, medical services, and waiver supports.
- Monitor individual health, safety, service quality, and continuity of care across home, day program, medical, and community settings.
Documentation and Compliance
- Complete person-centered plans, monthly QIDP notes, assessments, medical records, risk assessments, consent documents, and other required documentation accurately and on time.
- Maintain organized, current files that meet Helping Hand, DHS, OIG, ISC, and CARF standards.
- Follow applicable laws, professional standards, Social Security requirements, Public Aid regulations, and agency policies.
- Stay informed about regulatory and service-system changes that may affect individuals or agency services.
Training and Team Collaboration
- Provide guidance and training related to individualized plans, daily living skills, community integration, health and wellness, behavior supports, counseling plans, and leisure skills.
- Collaborate with supervisors, Case Management team members, and agency departments to address individual needs.
- Participate in biweekly supervision and attend required team and agency meetings and trainings.
- Contribute to a positive, inclusive, and equitable work environment while promoting Helping Hand's values.
- Other duties as assigned by supervisor.
WHAT YOU BRING:
Education:
- Bachelor's degree in Psychology, Social Work, or a related field required.
- QIDP certification (preferred).
Experience:
- One (1) year of experience working with individuals with intellectual and developmental disabilities.
Skill Sets:
- Strong organizational, communication, and problem-solving skills.
- Proficiency with Microsoft Office, Teams, Zoom, etc.
- Valid driver’s license with an acceptable driving record, ongoing personal motor vehicle insurance, and access to a reliable vehicle that can be used for work-related duties as required.
- Ability to pass PACE certification requirements (preferred).
- Ability to self-transport to work and home visits
WORK ENVIRONMENT:
- The person in this position may encounter clients exhibiting physical aggression; must be comfortable working in periods of crisis and feel comfortable de-escalating individually or as a team.
- The person in this position needs to occasionally move about inside the office to assist clients, access file cabinets, office machinery, etc.
- The ability to observe details at a close range (within a few feet of the observer).
- Consistently position self to maintain files in cabinets or assist clients.
- The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in all situations.
