This is a remote position, allowing you to work from your home while engaging with clients through virtual consultations. Key Responsibilities\n

• Conduct thorough assessments with clients to discuss their goals, preferences, and available treatment options before developing personalized rehabilitation plans.

• Design and implement individualized rehabilitation plans that take into account clients' education levels, aptitudes, physical capabilities, and aspirations for future employment, adjusting plans as needed to reflect their evolving needs.

• Collaborate with a multidisciplinary team to coordinate comprehensive rehabilitation programs tailored to each client's unique circumstances.

• Maintain meticulous case files and records that document clients' personal information, eligibility criteria, insurance details, services rendered, and ongoing contact narratives.

• Monitor and document client progress systematically to assess whether established goals are being met or require modification for continued success.

• Engage with clients during job training and occupational placement programs, maintaining contact to address any arising issues promptly.

• Work collaboratively with clients' family members and support systems to identify the most effective ways to meet each client's needs.

• Conduct timely evaluations of clients' needs and provide comprehensive documentation of findings and recommendations.

• Offer referrals and recommendations to additional resources that may enhance clients' recovery and support systems.

Patient Population / Client Focus\n

You will primarily work with clients experiencing various mental health challenges including mood disorders, anxiety, and other psychological conditions. Our client population ranges from individuals seeking help in critical transitional phases of their lives to those in need of ongoing mental health management. Each client presents unique circumstances, and your role will be essential in helping them navigate their journeys toward improved mental health.

Qualifications\n

• A master’s degree in counseling, social work, psychology, or a related field is required.

• Valid Licensed Professional Counselor (LPC) licensure.
