Key Responsibilities\n

• Conduct comprehensive assessments to determine the mental health needs of patients, tailoring interventions accordingly to promote effective treatment.

• Maintain open lines of communication with supervisors, family members, guardians, and other stakeholders regarding patients' progress and care strategies.

• Prepare and manage a schedule for appointments, home visits, and clinical observations, ensuring timely and effective treatment plans.

• Coordinate and execute home visits as needed, which involve chart reviews, staff interviews, and performance evaluations to ensure adherence to treatment strategies.

• Guarantee the effective implementation of the treatment program plan, modifying as necessary to align with patient progress and feedback.

• Actively participate in Interdisciplinary Team Meetings, contributing insights and posing solutions for enhancing patient outcomes.

• Document monthly and quarterly progress notes to maintain accurate records of client interactions and treatment developments.

• Facilitate training sessions for staff and team members to ensure competency in the implementation of program plans and documentation standards.

• Offer follow-up support through client interviews, home visits, and observations to ensure continuity of care for all clients.

• Serve as a liaison among the public and the mental health community, aiding in the development and execution of community outreach goals.

• Assist in creating and revising training materials to enhance program efficacy and ensure staff are well-equipped to support clients.
