Key Responsibilities\n

• Actively communicate with supervisors, families, and healthcare team members regarding clients' progress, facilitating continuity of care and the sharing of critical information.

• Develop and share a tentative appointment schedule with supervisors, ensuring timely follow-ups and the coordination of home visits.

• Conduct home visits to gather essential insights through observations, chart reviews, and interviews, ensuring an individualized treatment approach.

• Implement and monitor program plans with fidelity, adjusting strategies based on ongoing evaluations.

• Engage regularly in Interdisciplinary Team Meetings, bringing insights to patient care discussions and treatment decisions.

• Document monthly and quarterly progress notes to track patient improvement and care outcomes effectively.
\n
• Organize and facilitate training sessions for staff, ensuring they are equipped with the necessary skills and knowledge for successful implementation of program plans.

• Provide follow-up services through interviews and home visits to ensure sustained support for clients in their recovery journey.

• Maintain comprehensive records to ensure all information pertaining to patient care is readily available to relevant stakeholders.

• Serve as a liaison with the public and mental health community to promote awareness and understanding of our organizational goals.

Qualifications\n
