Key Responsibilities\n

• Conduct comprehensive evaluations to determine the unique needs and objectives of patients, ensuring personalized treatment plans are developed.

• Facilitate communication among interdisciplinary teams, including supervisors, support staff, family members, and stakeholders to provide updates on client progress.

• Develop and share progress tracking plans that outline appointments, home visits, and observation sessions aimed at achieving client goals.

• Organize and conduct home visits for needs assessments, observations, and data collection, ensuring compliance with treatment protocols.

• Implement program strategies effectively, ensuring accuracy in the execution of therapeutic interventions.

• Participate actively in Interdisciplinary Team Meetings (IDT) to exchange insights and foster a collaborative treatment approach.

• Prepare detailed monthly and quarterly progress reports, documenting client advancement and areas for continued focus.

• Lead training sessions for staff on the execution of treatment plans, emphasizing documentation practices and adherence to ethical standards.

• Conduct follow-ups through interviews and home visits, continuously assessing client requirements and adjusting care plans as necessary.

• Ensure client records are meticulously managed, maintaining confidentiality while facilitating access for relevant parties.

• Act as a liaison between the mental health community and the general public to enhance awareness and access to services.

• Collaborate on the development of educational materials that enhance our programs and promote client understanding.

Qualifications\n
