Key Responsibilities\n

• Collaborate extensively with primary care practitioners to identify, address, and manage patients' mental health and psychosocial issues in a manner that supports a cohesive treatment approach.

• Conduct thorough patient intakes that emphasize diagnostic and functional evaluations, utilizing the findings to develop targeted therapy objectives and treatment recommendations.

• Offer urgent mental health interventions to clients in crisis, ensuring timely support and access to additional resources as needed.

• Regularly assess patients' progress through follow-ups, modifying treatment plans based on ongoing evaluations of effectiveness.

• Develop and implement relapse prevention strategies and educational materials that empower clients toward better self-management of their mental health.

• Maintain comprehensive documentation of patient interactions, progress notes, and treatment plans to ensure compliance with Agency and Federally Qualified Health Center (FQHC) standards.

• Stay engaged with primary care providers, facilitating open communication regarding patients’ needs, treatment efficacy, and collaborative care strategies.

• Advise and guide patients in adhering to medical therapies and coping mechanisms for any side effects they may experience.

• Participate in team meetings to evaluate service delivery and contribute insights for program development and patient care strategies.

• Adhere to the Clinical Professionals' Code of Ethics, ensuring accountability and professionalism in all interactions.

Qualifications\n

• A Master's Degree in Clinical Counseling is required.

Compensation and Benefits\n

The salary range for this role is competitive, between USD 90,000 and 105,000 annually, reflecting the importance of mental health services in our organization.
