Key Responsibilities\n

• Conduct thorough and regular evaluations of patients to assess their mental health needs, developing tailored treatment plans based on individual assessments.

• Track and document progress using electronic medical record (EMR) software, including sharing tentative plans that encompass appointments, home visits, and observation sessions with supervisors.

• Organize and conduct home visits to carry out in-home assessments, chart reviews, and interviews as part of the treatment strategy.

• Implement and monitor program strategies, ensuring adherence to established therapeutic techniques and treatment methodologies.

• Participate in monthly interdisciplinary team meetings to collaborate on patient care and share insights that enhance program effectiveness.

• Generate meaningful progress reports on a monthly and quarterly basis to document outcomes and identify areas for improvement.

• Conduct additional follow-up interviews and home visits for ongoing evaluation and adjustment of individual treatment plans.

• Manage confidential patient records effectively, ensuring that all relevant parties have access to crucial information.

• Act as a liaison between the client population and the broader mental health community, promoting resources and development goals for better mental health access.
