This role is designed for a remote workspace aligned with our outpatient office operations. Key Responsibilities\n

As a Licensed Professional Counselor at Mohawk Trail Counseling Center, you will:

• Collaborate with primary care providers to identify and manage mental health and psychosocial issues, contributing as an integral member of the care team.

• Conduct patient intakes focusing on diagnostic evaluations and functional assessments, then develop treatment recommendations in coordination with primary care providers.

• Provide brief, evidence-based therapeutic interventions for clients requiring mental health services, adapting strategies to meet individual needs.

• Identify patients who may benefit from specialty mental health services and facilitate appropriate referrals.
\n
• Perform regular follow-up with patients to monitor progress and determine if adjustments to treatment plans are necessary.

• Develop relapse prevention strategies for patients, supporting them in maintaining stability.
\n
• Monitor and coordinate overall health service delivery related to behavioral health, linking patients with other treatment providers with their consent.

• Maintain accurate and comprehensive records of patient progress, treatment plans, and diagnostic information within our EHR system.

• Facilitate behavioral changes through patient education and self-management strategies, empowering clients to participate actively in their recovery.

• Consult with clinic management and team members on behavioral health service areas, contributing to program evaluation and improvement efforts.

• Assist patients in complying with medical treatments initiated by primary care providers, offering strategies to navigate medication management.

• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals.

• Perform any additional duties as assigned by management.

Compensation and Benefits\n

This position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
