Key Responsibilities\n

• Conduct thorough assessments to identify the mental health needs of each patient, utilizing evidence-based practices to inform treatment plans.

• Facilitate effective communication among supervisors, direct care staff, family members, and other stakeholders to foster a supportive environment for patient growth.

• Develop and provide a tentative schedule of appointments, home visits, and observations to monitor patient progress effectively.
\n
• Coordinate home visits and complete in-home observations, ensuring accurate data collection for ongoing treatment evaluations.
\n
• Ensure that the treatment plan is actively monitored and implemented, making adjustments based on patient feedback and progress.
\n
• Participate in Interdisciplinary Team Meetings to discuss patient care strategies and share insights with your colleagues.
\n
• Complete detailed monthly and quarterly progress notes that outline patient achievements and areas for improvement.
\n
• Organize staff training sessions to certify team members on program plan implementation and documentation practices.
\n
• Conduct follow-up assessments through interviews, home visits, and observations to provide ongoing support tailored to the patient's evolving needs.

• Maintain accurate records to ensure that essential information is readily shared among authorized parties involved in patient care.

• Act as a liaison between the clinic, the public, and the broader mental health community, advocating for resources and support for developmental goals.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Valid and unrestricted licensure as a Professional Counselor (LPC) within Arizona is mandatory.

• Experience or specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance addiction is strongly preferred.

• Comprehensive understanding of legal and ethical standards in mental health practice.

• Exceptional interpersonal and communication skills, with the ability to establish rapport with a broad range of clients.

• Proven ability to work collaboratively within a healthcare team to ensure cohesive patient support.

• Proficiency in using EMR software and other related technologies to document patient care and track treatment outcomes.

Compensation and Benefits\n
