JOBSUMMARY: Develops referral sources and partnerships through outreach within the community and with neighborhood leaders. This position promotes partnerships on local events. Identifying key partners and developing new relationships in high need communities.

ESSENTIAL FUNCTIONS:

1. Actively meets with community programs, leaders, and faith-based organizations to develop new relationships and partnerships.
2. Works with Program Manager to identify community-based relationships.
3. Completes timely documentation on outreach efforts and partnerships.
4. Ensures Program Manager is well-informed about outreach efforts.
5. Collaborates with neighborhood centers, faith-based organizations, and community stakeholders to identify target population.
6. Coordinates interagency communication and activities.
7. Actively monitors referral sources in quality improvement measures to help ensure that contracted service levels are being met.
8. Acts as a resource to neighborhood centers, faith-based organizations, community stakeholders, and collaborative partners.
9. Attends training, participates in internal/external committees, and attends all internal meetings as directed.
10. Works within the philosophy and function of the Children’s Home Network.
11. Performs other duties as assigned.

MINIMUM QUALIFICATION REQUIREMENTS:

Education & Experience: High School diploma or equivalent and two (2) years’ experience in Social Services environment or related field.

Licenses & Certifications:

• Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension.
• Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CHI’s current auto insurance policy.
• Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check and sexual predator screening.

PRIMARY CLIENTS SERVED:

Families identified as relative caregivers providing primary care for children ages 0-17 of kin that are self-referred or through community referrals.
