Work Environment\n

This remote role allows social workers to deliver exceptional service from a home-based office while being a pivotal part of our integrated care team. Although the position is remote, our Clinical Social Workers maintain regular communication with colleagues and collaborate with interdisciplinary teams, including medical staff and care coordinators, to ensure comprehensive support for hospice clients and their families.

Key Responsibilities\n

• Conduct thorough psychosocial assessments for patients referred to hospice care, gathering critical information regarding their mental health needs, life histories, and coping mechanisms.

• Provide tailored individual, family, and group therapy sessions using evidence-based modalities, including cognitive-behavioral therapy and trauma-informed care, to support various patient situations.

• Develop and implement individualized treatment plans, establishing clear goals and interventions that are aligned with the patient's unique circumstances.

• Collaborate directly with physicians, nurses, and chaplains to ensure coordinated and holistic care that addresses both physical and emotional aspects of end-of-life experiences.

• Facilitate group therapy sessions and psychoeducation workshops focused on bereavement, caregiver support, and adjustment to terminal illness, helping patients and families navigate their emotional journeys.

• Provide crisis intervention and risk assessment as necessary, ensuring patient and family safety under challenging circumstances.

• Maintain accurate and timely documentation of patient interactions, assessments, and treatment outcomes in accordance with ethical and legal standards, utilizing our electronic health record (EHR) system.

Patient Population / Client Focus\n

The hospice center primarily serves individuals facing terminal illnesses, along with their families, requiring emotional and psychological support. Our diverse client population includes patients with advanced cancer, neurodegenerative disorders, and other chronic conditions. The Clinical Social Worker will play a fundamental role in helping both patients and their families cope with grief, loss, and the emotional complexities surrounding end-of-life care.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

• A minimum of two years of relevant experience in a mental health or outpatient setting is essential, particularly focused on vulnerable populations such as those receiving hospice care.

Compensation and Benefits\n

This position offers a competitive salary range of USD 80,000 – 100,000 annually, complemented by a sign-on bonus. In addition, we provide an extensive benefits package that includes:
