Work Environment

This full-time position is offered in a remote work setting aligned with our hospice center's initiatives, enabling flexibility while ensuring you are a pivotal part of our interdisciplinary team. Regular business hours will be maintained, with occasional evening and weekend shifts necessitated by patient needs.

Key Responsibilities

• Conduct in-depth psychosocial assessments through patient interviews, observational techniques, and thorough reviews of medical records to assess emotional, social, and environmental factors affecting each patient's recovery and comfort.
• Provide compassionate counseling and emotional support to patients and their families, addressing adjustment issues, grief, anxiety, and general emotional well-being during a critical life phase.
• Collaborate closely with our rehabilitation team to develop and implement individualized treatment plans that reflect the unique psychosocial needs of patients, enhancing their overall hospice experience.
• Assist patients and families in navigating and accessing community resources, including support groups and vocational rehabilitation, ensuring a smooth transitional experience as they approach end-of-life discussions.
• Provide education to patients and their families about available community resources and post-rehabilitation services, empowering them with knowledge and support for future care.
• Play an active role in discharge planning, ensuring effective transitions from hospice to home or other care settings through comprehensive communication and coordination with all parties involved.

Patient Population / Client Focus

The patients you will serve encompass a diverse population facing life-limiting conditions, including those with advanced cancer, chronic health conditions, and neurological disorders. Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
