Clinical Coordinator - JSUMC Transfer Center - F/T Nights

About This Role Location: Neptune, NJ Facility: Hackensack Meridian Health Schedule: Full-time, Night shift What You’ll Do Answer telephones in accordance with established policies and procedures. Coordinate communication between sending and receiving facilities. Accurately record details of all communication related to each transfer request in the electronic health record. Collaborate with capacity management, nursing, physician/provider representatives, and other hospital departments. Maintain awareness of bed availability on all patient care units and outpatient departments. Handle a high degree of pressure, heavy workloads, and multiple requests. Diffuse challenging situations and respond appropriately. Implement solutions to transfer center issues independently. Follow patient identification policies and procedures. Collaborate with Emergency Medical Services (EMS) for timely transportation. Requirements Education RN graduate of an accredited school of nursing Experience At least 2 years of clinical experience in an acute care setting required Critical care and/or emergency nursing experience preferred Transfer center experience preferred BSN preferred Licenses & Certifications Current and valid New Jersey Registered Nurse license required BLS/CPR certification by American Heart Association required Hackensack Meridian Health is a leading healthcare provider committed to delivering exceptional patient care.
