Key Responsibilities\n

• Conduct thorough and ongoing assessments to identify the unique needs of each patient, tailoring therapeutic approaches accordingly.

• Facilitate effective communication amongst supervisors, direct care staff, families, and other stakeholders concerning individual client progress and treatment plans.

• Establish a tentative schedule that outlines appointments, home visits, and supervision meetings, ensuring organized and efficient service delivery.

• Coordinate and conduct home visits that complement in-home assessments, chart reviews, staff interviews, and the analysis of client data.
\n
• Ensure the seamless execution of the treatment program, advocating for patients and adjusting strategies as dictated by their ongoing needs.

• Participate in interdisciplinary team meetings and case discussions, contributing to collaborative decision-making for the best patient outcomes.
\n
• Document comprehensive monthly and quarterly progress notes that reflect treatment milestones and evolving patient support strategies.
\n
• Organize and conduct training for staff, ensuring competency in program plan implementation and adherence to documentation standards.
\n
• Perform follow-up sessions, including interviews and visits, to maintain continuity of care and address any emerging needs of clients.

• Maintain meticulous client records, ensuring that information is accessible and secure for fellow healthcare team members and stakeholders.

• Act as a bridge between the organization and the broader mental health community to support outreach and developmental initiatives.

• Assist in developing training materials that enhance program effectiveness and enrich client services.

Patient Population / Client Focus\n

In this role, you will work with a diverse range of clients, including both adults and youth, presenting with various mental health concerns. Your expertise will be critical in addressing issues such as anxiety, depression, trauma, and substance misuse, facilitating therapeutic processes that promote healing and resilience.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must hold active and unrestricted licensure as a Professional Counselor (LPC) in the state of New Jersey.

• A minimum of two years of counseling experience in a clinical context is essential.

• Experience or specialization in family therapy, couples therapy, trauma-informed care, or substance misuse will be highly regarded.

• Possess a solid understanding of legal and ethical standards within the mental health profession.
