Requirements ***Client has 8hrs of Non-Billable hours for orientation hours *** At time of Offer please have provider's DOB SSN ready to submit*** as this is required to present to client within 24hrs of an offer. *Not requiring ERCP but if providers who have it are interested please let us know. If presenting with ERCP call it out in the PRESENTATION SUBJECT line AND PRESENTATION FORM START- ASAP ~90 day cred timeframe. DURATION- 1/14/27 approval expires possibility of extension FTE Need- .5 1 FTE approval but this is shared among other locums providers confirmed they have .5 FTE still needed WHY OPEN?- Gap in coverage Will work at a secondary clinic nearby Types of Privileges - Clinic Hospital Setting- Both inpatient outpatient # of Beds 251 ROUNDING- Admissions and procedures- Yes Patient Demographics- Adult 18 Patients per day- 16 outpatient endoscopy center 14-16 hospital census- varies Required Procedures- Colonoscopy EGD Flex Sig Dilation General GI Procedures REQUIRED CLINICAL ACTIVITY IN LAST 24 MONTHS- Must provide documentation of provision of gastro services representative of the scope and complexity of the privileges requested during the previous 24months waived for applicants who completed training during the previous year AND If training to perform double balloon enteroscopy or capsule endoscopy occurred in the previous 2 years applicant must provide a description of supervised training on human subjects along with a letter from the supervising physician attesting to clinical competence as an independent operator. If training occurred greater than 2 years ago the applicant must provide documentation of ongoing clinical practice in each privilege requested. Provider will work w/ another GI MD that is part of the practice LICENSE- WA licensed or IMLC ONLY Certs required- BLS ACLS EMR- EPIC Shift Info: Day 5x8-Hour 08:00 - 17:00
