Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the mental health needs of patients, adjusting treatment plans to ensure optimal care.

• Regularly communicate with medical professionals, family members, and guardians to discuss patient progress and readjust care strategies as needed.

• Maintain a schedule that includes appointments, home visits, and necessary follow-ups, ensuring all patient interactions are adequately documented.

• Coordinate home visits to perform in-home assessments, which include observing patient interactions and reviewing relevant documents.

• Ensure program plans are effectively implemented and adhered to, offering insights to enhance treatment methodologies.

• Participate in Interdisciplinary Team Meetings (IDT) to collaborate on patient care and share critical updates on treatment initiatives.

• Complete regular progress notes and comprehensive documentation on patient status, ensuring transparent and accessible records.

• Develop and lead training sessions for staff members on program plans and documentation requirements to ensure consistent quality across teams.

• Conduct follow-up interviews and home visits to ensure continuous patient support tailored to their evolving needs.

• Act as a key liaison between the organization and the public mental health community to foster development goals and improve community awareness.

• Assist in creating engagement materials to enhance training programs and presentations that benefit individual and team efficacy.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required for this role.

• An active and unrestricted Professional Counselor (LPC) license in the state of Arizona is mandatory.
