Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, followed by recommendations to primary care providers for treatment goals.

• Provide brief, targeted interventions for patients in need of mental health services, emphasizing evidence-based therapeutic techniques.

• Collaborate with primary care providers to identify which patients are best served in the primary care setting versus those needing referrals to specialty mental health services.

• Document and monitor patients' progress in treatment charts, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.

• Develop and implement relapse prevention plans and assist patients in maintaining stable functioning in their daily lives.
\n
• Work to identify at-risk patients and formulate strategic plans to prevent the worsening of their conditions.
\n
• Coordinate the delivery of behavioral health services and facilitate connections with other treatment providers, both within and outside the clinic, with patient consent.
\n
• Engage with patients to provide self-management tools and educational information, empowering them to take an active role in their treatment.
\n
• Consult with clinic management and team members regarding behavioral services and provide insights on program evaluation.
\n
• Assist patients in aligning their medical treatment with psychological support, offering strategies for coping with any medication side effects.

• Maintain adherence to the Code of Ethical Conduct for Clinical Professionals in all interactions.

Qualifications\n

• Ability to apply proficiency in DSM-IV diagnoses and psychodiagnostic assessments effectively.

• Strong interpersonal skills to develop cooperative relationships with patients, families, and colleagues.

• Capacity to maintain detailed clinical records and prepare comprehensive reports.

• Ability to thrive in an interdisciplinary team setting, contributing to holistic care approaches.

Preferred Experience\n

• Fluency in both English and Spanish is preferred, but not mandatory, to better engage with our diverse clientele.

Compensation and Benefits\n

This full-time licensed professional counselor position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
