Position Title Licensed Master Social Worker (NY HELPS), Hutchings Psychiatric Center, Mobile Integration Team (P27216) Location 620 Madison Street, Syracuse, NY 13210 Responsibilities Provide clinical and support services in the community for individuals with mental illness, helping them maintain independent living in their homes or community settings. Deliver brief therapeutic support using evidence‑based practices to teach symptom‑management, coping skills, medication management, and crisis reduction. Develop and implement community‑based groups to support and improve functional outcomes. Offer case‑management services, linking individuals to community services, coordinating with other providers, involving families, advocating, and accompanying clients to community appointments. Collaborate with clinic staff to perform wellness checks on individuals who are lost to contact or difficult to engage. Maintain required documentation: daily contact notes, service records, GOMPS, individual service plans and reviews, progress notes, referral forms, and inter‑agency correspondence. Provide crisis intervention, service linkages, and discharge planning. Develop treatment plans with clients and the treatment team, monitor progress, and modify plans as necessary. Determine discharge needs and identify appropriate community resources. Establish and maintain relationships with community providers to ensure continuity of care. Qualifications Licensed Master Social Worker (LMSW) registered by the State of New York. For candidates outside or within State Service: must hold an active LMSW license. State Service candidates may transfer to a competitive class position if they have one year of permanent competitive or 55‑b/55‑c service in a title eligible for transfer under Section 70.1 of the Civil Service Law. Benefits and Compensation Salary Grade 20: $74,193 to $94,121 annually. Shift differential pay: 7% for evening and weekend day shifts, 10% for night shifts.
