Nurse Practitioner

Job Summary We are seeking a Per Diem Nurse Practitioner to work with our hospice patients in the Chester County area. This is an expansion area for our program and we are excited to be part of the Chester County area. This position is Per Diem, therefore no guaranteed hours. Responsibilities Work in accordance with the prescribed policies and procedures of the organization, including all applicable state and federal regulations. Perform patient medical examinations and palliative care visits required in order to certify patients’ eligibility for the Medicare Hospice Benefit after six (6) months of hospice care. Document visits in the patient’s clinical record and complete required forms for face‑to‑face encounters. Provide medical care services and palliative care intervention for hospice patients on an as‑needed basis. Communicate effectively with the Medical Director, Administrator and the Hospice Interdisciplinary Team. Basic Qualifications Graduation from a master’s‑level program designed to educate and prepare nurse practitioners/clinical nurse specialists. Completion of a graduate level 3‑credit course in Pharmacology from a school duly accredited by a national accrediting agency approved by the Board or a pharmacology course of at least 45 hours integrated into the Master’s in Nursing program. National certification requires the candidate to successfully pass an examination. Currently licensed and nationally certified via the PA Board of Nursing as an Advance Practice Nurse in PA. Valid driver’s license and reliable transportation that is insured in accordance with PA requirements. Skills Provide services in a timely and professional manner in accordance with hospice operational policies and procedures, including the Compliance Program, billing submission, Medicare Conditions of Participation, PA Licensure Guidelines, and applicable laws. Demonstrate a positive, professional manner, manage time efficiently and effectively, work with a team approach, and act as a role model. Develop and maintain effective relationships with the interdisciplinary group, other professionals and physicians in the community. Show interest, compassion, and a sympathetic attitude toward caring for patients in a hospice care setting. Maintain a professional attitude and demeanor, and communicate effectively and courteously with patients, caregivers, colleagues, supervisors, co‑workers and others. Demonstrate optimism, enthusiasm, and willingness to work constructively with other health care team members and reflect concern for the wellbeing of patients and employees. Interact positively with all staff in support of the organization’s mission and vision. Experience Minimum of two (2) years’ experience as a Nurse Practitioner.
