Too often, behavioral health needs go untreated because access to care is limited. At High Rock Internal Medicine, we believe mental health is healthcare. That’s why we’re looking for a compassionate Behavioral Health Care Manager to help bridge the gap between physical and behavioral health services. In this role, you’ll become a trusted partner for patients who need support navigating life’s challenges while improving their health outcomes. Working within the Collaborative Care Model (CoCM), you’ll have the opportunity to make a meaningful difference in patients’ lives by providing evidence-based interventions, coordinating care with medical providers and psychiatric consultants, and helping patients achieve healthier, more fulfilling lives.
