Registered Nurse Case Manager - Hospice

Job Overview We are seeking a dedicated Hospice RN to join our team in Gainesville, FL. This full‑time position runs Monday through Friday, 8a-5p, with no weekends or holidays. Coverage includes Alachua and Bradford Counties, primarily Newberry and High Springs. Location Gainesville, FL (Coverage area: Alachua, Bradford Counties) Responsibilities Provide 1:1 patient and family–centered care in hospice settings. Assess, monitor, and document physical, emotional, and psychological needs. Create and implement hospice care plans aligned with patient goals. Administer medications, treatments, and interventions. Manage pain and other symptoms. Educate and support patients’ families and caregivers. Collaborate with interdisciplinary team members. Maintain accurate, timely documentation. All benefits may vary by employment status. Qualifications Graduate of an accredited nursing school with current licensure to practice in Florida. Minimum one year nursing experience; three or more years preferred. Experience in hospice or a similar setting preferred. Knowledge of the hospice philosophy of care. Commitment to clinical and documentation excellence. Licensed driver with an insured automobile in good working order.
