Job Description

We are seeking a Skilled Home Health Nurse with a minimum of 1 year of experience in home health care. The ideal candidate will provide skilled nursing visits for patients on Medicare, Medicare Advantage, and commercial insurance plans. Responsibilities include performing cardiopulmonary assessments, wound care, and other nursing duties as outlined below.

Responsibilities

• Conduct comprehensive assessments and evaluations of patients' health status.
• Administer medications and treatments as prescribed, including IV administration and wound care.
• Manage foley catheters, wound vacs, and perform staple/suture removals.
• Utilize medical equipment such as glucometers and pro-time machines.
• Provide patient education on disease management and self-care.
• Document patient interactions and visit details accurately and timely using BayCare Forms and scheduling systems.
• Work collaboratively with an interdisciplinary team including physical therapy and social services.

Required Experience / Certifications / Licensure

• Minimum 1 year of skilled home care experience, excluding private duty nursing.
• Experience with OASIS documentation and competencies in IV skills and wound care.
• Current BCLS certification required.
• Valid driver's license and reliable transportation for home visits.
• COVID-19 vaccination or approved exemption; must wear a mask while working.
• Flu vaccine required or declination subject to mandatory masking.
