This position operates predominantly in a remote setting, allowing for a flexible work-life balance. Your primary interaction will be with clients who may require assistance via telephone, video conferencing, or face-to-face meetings as necessary. While engaging remotely, you will access the latest technology and digital systems to maintain connectivity with clients and report relevant case notes efficiently. Key Responsibilities\n

• Plan and implement individualized support packages to address client challenges, with a focus on overcoming dependencies and strengthening coping skills.

• Conduct thorough interviews with clients, assessing their current mental and emotional status, strengths, and areas requiring support.

• Develop tailored, measurable goals for each client case and regularly monitor their progress, adjusting treatment plans as needed to ensure effectiveness.

• Provide crisis intervention and supportive counseling, offering clients various options based on their unique situations.

• Maintain and document accurate client records, monitoring their ongoing status and ensuring compliance with regulatory standards.

• Participate in case conferences with multidisciplinary teams, contributing insights, and offering evidence when necessary, particularly in collaboration with court proceedings.

• Act as a key contact for clients, coordinating with various community service agencies to assist in their recovery journey.

Patient Population / Client Focus\n

This role primarily involves working with clients navigating complex social and mental health issues, particularly those receiving hospice care. Compensation and Benefits\n

The compensation for this rewarding role ranges from USD 75,000 to 90,000 annually, coupled with a sign-on bonus for qualifying candidates.
