Key Responsibilities\n

• Conduct comprehensive assessments to evaluate clients’ mental health status and needs.

• Develop tailored treatment plans that align with client goals, ensuring a personalized approach to care.

• Provide supportive individual, group, and family counseling sessions, using techniques such as cognitive-behavioral therapy (CBT) and mindfulness-based stress reduction.

• Monitor and document client progress towards treatment goals, making necessary adjustments along the way.

• Collaborate with a multidisciplinary team of healthcare providers, including psychiatrists and psychologists, to deliver an integrated care experience.

• Maintain detailed, up-to-date records of client interactions and treatment outcomes using our electronic health record (EHR) system.

Patient Population / Client Focus\n

Your primary focus will be on clients experiencing grief or emotional struggle related to terminal illness, as well as those facing significant life transitions. This includes support for individuals dealing with loss, relationship changes, and mental health difficulties such as depression and anxiety. You will play a critical role in helping these individuals and their families navigate the complexities of their situations, providing compassionate and effective counseling tailored to their unique experiences.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.
