Key Responsibilities\n

• Conduct thorough assessments and evaluations to understand the mental health and psychosocial needs of clients dealing with end-of-life issues.

• Develop and implement individualized treatment plans utilizing evidence-based practices tailored to each client’s unique circumstances and preferences.

• Facilitate individual therapy sessions via our telehealth platform, ensuring that clients have access to necessary care regardless of location.

• Actively collaborate with clients to establish therapeutic goals and continuously monitor their progress, making adjustments to treatment plans as necessary.

• Maintain meticulous documentation of all client interactions, treatment plans, and progress notes to adhere to ethical standards and regulatory requirements.

Patient Population / Client Focus\n

As a Remote LCSW, your primary focus will be on clients within the hospice care setting. This includes providing emotional support and mental health interventions to patients who are facing serious illnesses as well as their families. Qualifications\n

• A Master’s degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).
