Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews and integrative observations, ensuring a comprehensive understanding of patient needs and circumstances.

• Participate in complex discharge planning, coordinating community services and resources to ensure patients transition smoothly from hospice to needed support services.

In this role, you will primarily support patients receiving hospice care, who may be coping with life-limiting illnesses and associated psychosocial stressors. Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current and active LCSW license in California.

Compensation and Benefits\n

We offer a competitive annual salary ranging from USD 120,000 to 130,000, reflecting your qualifications and experience.
