Key Responsibilities\n

• Perform thorough psychosocial assessments of clients focusing on emotional, mental, and social factors, integrating insights into overall care planning.

• Collaborate closely with clients and their families to create personalized care plans that address specific physical, emotional, and social needs.

• Provide counseling and emotional support to seniors and their families, helping them navigate significant life transitions, including health-related changes.

• Advocate for clients, ensuring they access necessary community resources and support systems, while also educating them on available services.

• Work in tandem with interdisciplinary teams to develop holistic care approaches, participating actively in case discussions and treatment planning.

• Conduct regular remote check-ins and monitoring visits to assess client well-being and address emerging concerns effectively.

• Facilitate constructive communication among family members involved in the care process, assisting in conflict resolution to support client interests.

• Maintain comprehensive and accurate documentation of client interactions, assessments, care plans, and progress notes in compliance with regulatory standards.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Must possess a current and valid state licensure as a Clinical Social Worker (LCSW).
