Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, evaluating emotional, mental, and social factors that influence their well-being.

• Develop and implement personalized care plans in partnership with clients and their families, focusing on their specific goals and preferences.

• Provide emotional support and counseling to clients, helping them navigate life transitions, bereavement, and other challenges they may face.

• Advocate for clients by identifying and facilitating access to community resources and support services, ensuring their rights and needs are met.

• Collaborate with an interdisciplinary team including healthcare professionals, caregivers, and family members to enhance the scope of care provided to clients.

• Conduct regular check-ins and home visits when necessary, to monitor clients' conditions and adjust care strategies accordingly.

• Educate clients and families on available community programs, support groups, and resources to promote social engagement and mental health.

• Maintain accurate and timely documentation of client interactions, assessments, treatment plans, and progress notes using an electronic health record (EHR) system.

• Stay updated on best practices, policies, and regulations relevant to social work, ensuring compliance with ethical standards.

Patient Population / Client Focus\n

This role primarily focuses on senior clients and their families, addressing a variety of psychosocial issues including isolation, depression, anxiety, and difficulties coping with aging-related challenges. The Social Worker will provide support to clients from diverse backgrounds and will need to be sensitive to the cultural and social factors affecting their care.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
