Key Responsibilities\n

As a Senior Licensed Clinical Social Worker, you will take on several key responsibilities, including:

• Conducting comprehensive mental health assessments, including psychosocial evaluations and diagnostic interviews, to understand each patient's unique needs.

• Collaborating closely with patients and families to develop individualized treatment plans that reflect their goals and cultural values.

• Providing evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), tailored for individual, couple, family, and group settings.

• Offering timely crisis intervention services and acting as a vital part of the multidisciplinary team to ensure a seamless continuum of care.

• Maintaining accurate and up-to-date clinical documentation, including progress notes, treatment plans, and discharge summaries, in compliance with our clinic's policies.

• Empowering patients and their families through psychoeducation, equipping them with the tools needed for effective mental health management.

• Regularly conducting outcome assessments to evaluate treatment efficacy and adjusting treatment plans accordingly to achieve optimal results.

• Staying abreast of the latest research, trends, and best practices in mental health to continuously enhance your clinical skills.

• Upholding the highest ethical standards and ensuring the confidentiality of patient information at all times.

Patient Population / Client Focus\n

This role primarily focuses on serving patients facing serious illnesses and their families within a hospice setting. You will engage with clients who may be experiencing grief, anxiety, fear, and existential concerns. Your role will be crucial in helping them navigate their emotional landscapes and providing comfort during challenging times. Your capacity to foster a safe space and build rapport will be essential in delivering effective psychosocial support.

• A Master's degree in Social Work from an accredited program.

Compensation and Benefits\n
